During this session Doctor Joelle Vlahakis will share results of the Centers for Medicare and Medicaid Services (CMS) Innovation Challenge Grant project to promote the use of Chronic Care Management, September 1, 2022 through August 31, 2023. She will highlight successes in Michigan and expand upon the activities of the team, including lessons learned and insights gained, with a focus on advancing health equity.
