 Upcoming Adelaide PHN commissioning opportunity

The Adelaide PHN will be requesting proposals from service providers for the provision of Local Area Coordination services for the Adelaide PHN Care Connections Program in the coming weeks.
The Adelaide PHN Care Connections Program is an exciting opportunity to use  elements of the Patient Centred Medical Home (PCMH) Model as a framework to improve chronic condition management. Care Connections will enable General Practices to test elements of the PCMH model at their own pace and with dedicated support, in the form of Local Area Coordination.

After-Hours Innovation Grants

In May, Adelaide PHN undertook a process for After Hours Innovation Grants. The process is now complete and the successful applicants have been advised. A number of high quality applications were received and we appreciate the effort that went into the applications. The projects commenced in August/September 2016 and will run until 30th June 2017. 

The purpose of the grants is to provide financial incentive to support GP practices and other primary care providers to implement innovative approaches to after-hours care.

National Shingles Vaccination Program

The Zostavax® vaccine is a free vaccine now available for people aged 70 - 79 years old. A single catch-up does will be funded for adults aged 71-79 years until 31 October 2021. Zostavax® is recommended, but not funded, for people 60 years and over as this age group is at an increased risk of shingles and its complications.People who are not eligible to receive the free vaccine are able to purchase the vaccine on the private market.
